To explain how to set this up I have copied the explanation out of the Revit. Since it is part of the standard installation, do not change it. Any changes you make may be lost when you update Revit. To define custom patterns, create a new file using this file as a template.
These are rather elaborate title blocks. To quickly adjust the title block of your sheet: On the Home Tab there are various options to add components to your title block.
When you're done, click the Load into Project button in the Ribbon. This will overwrite the Title block family that Revit loaded from disk for this Project only.

You can access sheet parameters that are set in the Project by adding a 'Label instead of a regular text field. Go to the Create Tab, Text section and click Label. Place your text field. Afterwards the following window appears: Choose which parameters you want to use in the text field.

So I click "Add ", I type in ARCH D for the paper name, I type in 36 for the width and 24 for the height. The inch unit check box is checked.
